My high school ensemble teacher did something like this. He gave us a study guide then an exam. The test covered voice parts, types of instruments (which clef do they read, in what type of piece would you hear / use a given instrument); note-reading of course: treble and bass clefs; musicianship skills such as what to do when you are sightreading a piece, what tempo to take the piece. There were questions about triads and major/minor keys. I think there were questions about the pieces we had played that year – he always taught background info. You might consider a listening section where you play a few examples then ask questions about instrumentation / voices, mood of the piece, style, etc.
